Urgent Community Response (UCR) - Mid Notts

Clinical

Urgent Community Response provides a rapid assessment service for adults in their own homes within Nottinghamshire, who are experiencing a physical health crisis, with the aim of preventing unnecessary ambulance call outs, attendances at A&E and admissions to hospital.

The service responds to the most high risk patients within 2 hours of referral, 7 days a week, 8.00 am-10.00 pm and often assesses and treats patients in distressing and potentially life impacting situations. All patients are seen in their own normal place of residence; which can be extremely variable in terms of environment. Patients can often be acutely unwell, injured or in the last stages of life.

The service works closely with A&Es across the county as well as EMAS, GPs, social care, community health services and all other health and social care providers.

On this work experience placement you well see how we triage and allocate referrals and learn about about the team from the clinicians who see the patients, with the possibility of attending patient visits should this be appropriate.

The service runs 8.00 am-10.00 pm 7 days a week, however we will offer a placement Monday to Wednesday 9.00 am - 5.00 pm. We can be flexible if this is needed, please state this on your application.

This initial placement offer covers the Mid Notts area (Mansfield, Ashfield, Newark and Sherwood) but we may also offer South Notts and Bassetlaw if this is requested.

**Please note that there may be a requirement to travel in Trust staff member's vehicles as part of this work experience placement**

One placement every 6 months

9.00 am - 5.00 pm

As this is a clinical placement, we ask that you attend wearing clothing that broadly matches our Trust policy. You will need to wear comfortable but smart clothes. Plain trousers and polo shirt (black, blue or white) with plain dark trainers or flat shoes are recommended. No jeans, highly branded clothing, shorts, cropped tops or heeled shoes.

Please ensure long hair is tied back, no false nails, gels or polish, no rings or large earrings.

Being able to drive is an advantage but not essential. We do ask that you arrive promptly to minimise impact on the team. If you can't attend a planned placement, please inform the team as soon as possible.

We ask that candidates think carefully about how they feel about being exposed to difficult and often distressing patient cases. Candidates can expect to hear about and potentially see patients in upsetting circumstances. There may be exposure to bodily fluids of all kinds, as well as patients with injuries and wounds. Some of our patients are at the end of their lives. All our patients are at risk of hospital admission.

We recommend this work experience for candidates hoping to work in emergency settings such as emergency departments, urgent treatment centres, ambulance services and urgent response services.
