Children and Young People - Healthy Family Team

The Healthy Family Team is made up of many different skill mixes who deliver an integrated 0-19 public health nursing service for children, young people, and their families. The team includes Screening Assistants, Healthy Child Assistants, Assistant Public Health Practitioners (APHPs) Public Health Practitioners (PHP) and Specialist Community Public Health Practitioners (SCPHP). The team provide advice, care and interventions offering a joined-up service to all families in Nottinghamshire.

There are locally based Healthy Family Teams across the County who deliver the programme. The service is provided to any family living in Nottinghamshire with a child between 0-19 years old, expectant mums and for young people attending Nottinghamshire schools up to the age of 19 years.

The Health Family Team provide a professional public health service based on the best evidence of what works for individuals, families, groups, and communities; enhancing health and reducing health inequalities through a proactive, universal service for all children aged 0 to 19 years.

Health visitors and school nurses are Specialist Community Public Health Nurse’s (SCPHN), with health visitors primarily leading the 0 to 5 elements of the Healthy Child Programme and school nurses leading the 5 to 19 elements. Whilst both services are led by health visitors and school nurses as SCPHN, there is also a skill mix within the team, that supports across the 0-19 age group.

All services and interventions are personalised to respond to children and families’ needs across time. For most families most of their needs should be met by the universal offer, while more targeted and specialised evidence-based support will be provided as early as possible.

This placement is a Trust wide placement depending on area and will be facilitated at multiple bases.

Reporting instructions will differ for each placement depending on locality

This will need to be assessed at the time of request. The service can offer numerous placement areas but it will depend on capacity at the time of request to how many other students are on placement from higher education settings.


Monday - Friday 0900-1700 Some areas do differ and placement hours will be agreed on an individual placement


Smart clothing - No uniform required unless they opt to wear one that is appropriate.


Young person should be in year 12 or above for work experience.
They can be 16 years of age but in year 12 studying.
